What it is
Retatrutide is an investigational peptide with agonism at three metabolic receptors: GIP, GLP-1, and glucagon. It is being developed for sustainable weight reduction, glycemic control, and improved metabolic efficiency. Phase 2 data suggest weight-loss magnitude exceeding tirzepatide; Phase 3 TRIUMPH trials are ongoing.
Mechanism of action
Adds glucagon receptor agonism to the dual-incretin approach. Glucagon receptor activation increases energy expenditure and hepatic fat oxidation, potentially compounding caloric-deficit effects from GLP-1 / GIP appetite suppression and insulin enhancement.
Half-life: ~6 days.
Evidence summary
Phase 2 (NEJM 2023) showed mean 24.2% weight loss at 48 weeks with 12 mg weekly. Subsequent body-composition and meta-analytic data (2025) support the magnitude of effect across BMI ranges.
Typical dosing protocols
- Starting dose: 2 mg subcutaneous weekly
- Escalate by 2 mg every 4 weeks as tolerated
- Maintenance: 4–12 mg weekly
- Slow titration recommended due to additive GI side-effect profile
Side effects & safety
- GI effects (nausea, vomiting, diarrhea) prominent during escalation
- Elevations in heart rate reported at higher doses
- Not yet FDA-approved — long-term safety under investigation
